Responsibilities

→Deliver acute, hospital-level care in patients’ homes
→Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
→Monitor patient conditions and recognize changes requiring escalation
→Collaborate with a virtual and in-person interdisciplinary team
→Use evidence-based practice to ensure safe, patient-centered care
→Coordinate discharge planning and ongoing care needs
→Promote a culture of safety and high-quality outcomes

Active RN license in the state of Illinois
BLS certification (AHA)
Valid driver’s license with a clean driving record
2+ years of recent acute inpatient care or ED experience preferred
Strong IV, phlebotomy, and clinical assessment skills

Ability to work independently in home settings
Excellent communication and critical thinking
Comfortable with technology and remote monitoring tools

Travel within designated service area
Provide care in private home settings
Physical activity required (lifting, bending, standing, etc.)
Exposure to clinical and environmental variables typical in-home care
