Path Assist is our tech-enabled community health worker program for HRSN utilizing an evidence-based, structured intervention. Our goal is simple: increase health confidence, improve self-efficacy, and reduce inappropriate healthcare spending. 

Activate Care is seeking a Clinical Support Supervisor to provide clinical guidance and escalation support to Community Health Workers delivering the Utah LIFT Path Assist for Chronic Disease program. This role will help ensure that CHWs are well equipped to support patients living with diabetes, hypertension, obesity, COPD, asthma, cardiovascular risk, and other chronic conditions while remaining strictly within the appropriate non-clinical CHW scope of practice.

The Clinical Support Supervisor will provide case consultation, reinforce evidence-based chronic disease self-management approaches, support clinical escalation protocols, strengthen coordination between CHWs and participating FQHC clinical teams, and contribute to training and quality improvement.

Responsibilities

  • →Serve as a clinical resource and provide structured case consultation for CHWs when participant needs, reported symptoms, or questions exceed routine non-clinical care-coordination support.
  • →Help CHWs distinguish between appropriate coaching/navigation activities and issues requiring clinical escalation, reinforcing provider recommendations without providing independent diagnosis, treatment, or medical advice.
  • →Provide subject-matter expertise related to common LIFT priority conditions, including diabetes, hypertension, obesity, COPD, asthma, and cardiovascular risk.
  • →Support standardized CHW workflows for chronic disease education, telehealth access, appointment follow-through, and clinical escalation.
  • →Coach CHWs in using motivational interviewing, goal-setting techniques, and approved WebMD Ignite educational content to support patient self-management behaviors.
  • →Develop, maintain, and reinforce clinical escalation protocols in collaboration with participating FQHCs, ensuring pathways identify appropriate clinic contacts and response processes.
  • →Guide CHWs on when and how to escalate symptoms, medication concerns, abnormal patient-reported information, or safety concerns.
  • →Support review of safety events, escalation failures, or near misses, recommending corrective actions and maintaining clear boundaries between CHW services and licensed clinical care.
  • →Build strong working relationships with FQHC clinical leads, providers, nurses, and care-management staff to align Path Assist activities with clinic workflows and provider-directed plans of care.
  • →Facilitate communication with FQHC clinical teams when patient needs require clinical escalation, participating in case conferences, workflow discussions, and quality-improvement meetings.
  • →Deliver or support CHW training on chronic disease fundamentals, clinical red flags, escalation pathways, medication-support boundaries, health literacy, and telehealth preparation.
  • →Develop practical case-based training scenarios and review chronic disease educational materials and workflows for clinical appropriateness.
  • →Support workflows for preparing patients for provider-delivered telehealth encounters and reinforce appropriate post-visit follow-up instructions.
  • →Review program trends related to clinical escalation, appointment follow-through, telehealth completion, and chronic disease barriers to drive quality-improvement activities.
  • →Collaborate with Data and Evaluation teams to review program performance, safety indicators, and clinical quality measures.
  • →Maintain strict compliance with HIPAA, privacy, security, documentation, and organizational policies.
  • →Other duties as assigned.

Requirements

  • Current, active RN license (or other appropriate clinical licensure based on program design) required.
  • Demonstrated clinical experience supporting adults with chronic conditions such as diabetes, hypertension, cardiovascular disease, obesity, COPD, or asthma required.
  • Experience in care management, population health, primary care, chronic disease management, or similar clinical programs required.
  • Strong understanding of clinical escalation, patient education, health literacy, and interdisciplinary care coordination required.
  • Demonstrated ability to translate complex clinical concepts into practical guidance for non-clinical staff required.
  • Strong coaching, communication, case-consultation, and multidisciplinary collaboration skills required.
  • Experience in an FQHC, Medicaid, or rural healthcare environment preferred.
  • Experience supervising, training, or providing clinical support to CHWs, care coordinators, health coaches, or patient navigators preferred.
  • Familiarity with telehealth-enabled chronic disease management, motivational interviewing, and population-health/care-management technology preferred.
  • Ability to travel to Utah clinic locations as needed; valid driver's license and reliable personal transportation required.
