Three 12-hour shifts per week
Includes every other weekend rotation

Responsibilities

→Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
→Conduct comprehensive nursing assessments and implement individualized plans of care
→Deliver skilled nursing services focused on comfort, dignity, and quality of life
→Administer and monitor pain and symptom management interventions
→Educate patients and families on disease progression, care plans, and symptom control
→Document patient care accurately and timely within electronic medical records (EMR)
→Collaborate with the interdisciplinary team to ensure coordinated, holistic care
→Evaluate patient responses and adjust care plans as needed

Requirements

Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
Current LPN/LVN license in the state of Ohio
Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
Current BLS certification
Strong clinical, communication, and organizational skills
Valid driver’s license, reliable transportation, and proof of insurance
Understanding of hospice philosophy and end-of-life care

Experience in hospice or palliative care
Familiarity with home-based care and EMR systems
