Location: 1724 N Mills Ave Suite 400 Orlando, Fl 32803

Schedule: Monday – Friday, 9:00 - 6:00PM and 8:30 AM – 5:30 PM as needed. Mainly 9:00 AM -6:00 PM

Triages incoming calls from facilities, patients, physician offices, hospitals, pharmacies, and home health. Coordinates new patient appointments and disseminates information based on acuity level. Educates and screens all new patients in preparation for their appointments to ensure their plan of care is appropriate. Navigates patients through their entire plan of care. Other duties as assigned.

Certain positions are subject to Florida Level 2 background screening, including fingerprinting, as required by state law.
