Responsibilities

  • Completing Start of Care (SOC) visits with confidence, compassion, and clinical excellence
  • Performing assessments that actually reflect the patient’s needs
  • Creating individualized care plans that set the tone for amazing outcomes
  • Educating patients and families so they feel empowered, not overwhelmed
  • Collaborating with a supportive team that values your judgment and expertise
  • Documenting in the EMR (we keep it as smooth and painless as possible)
  • Being the RN who makes patients say, “Wow… I feel better already”
  • Active RN license in Michigan
  • Strong assessment skills and clinical judgment, including experience conducting OASIS assessments and follow-up visits
  • Strong communication skills, including educating patients and families, explaining care plans, and handling Start of Care conversations
  • Home health and hospice experience
  • Experience collaborating with physicians and therapists
  • Reliable transportation and eligibility to work in the United States
  • Experience with Kinnser is a plus
