The Community Health Worker (CHW) is responsible providing support to patients and families as part of Rhode Island Primary Care interdisciplinary care team within a primary care setting. CHWs will be assigned to work with their assigned primary care and pediatric practices to coordinate care for a patient’s social determinant of health (SDOH) needs. The RIPCPC Community health workers are an integral part of the care management team. In this role, assessment of patient bio-psychosocial strengths and challenges will be integral in supporting their healthcare needs and conditions. As a CHW you will coordinate with RIPCPC interdisciplinary care teams and community-based organizations to bridge the gap in services for our patients.
Responsibilities
~1 min read- →Complete initial assessments with the patient and/or family member to identify specific areas of patient strengths and barriers to care.
- →Work to build a trusting relationship through continued rapport building techniques.
- →Empower patients to manage their care through motivational interviewing and goal setting.
- →Coordinate and collaborate with interdisciplinary care team members at RIPCPC, family members, and healthcare providers.
- →Advocate for patients and families to ensure the patients needs are represented and supported by their healthcare team.
- →Documentation in EPIC and native electronic medical record of physician practice.
- →Assist with setting up social services through a community-based organization including:
- →Food assistance o Housing transitions
- →State and federal applications, insurance, and benefits
- →Long term care resources
- →Child and family resources
- →Understanding social determinants of health (SDOH) and how they effect a patient’s health.
- →Collaborate with Behavioral health Clinicians as needed for patient care and improving patient outcomes.
Requirements

- Bachelor’s degree in a health-related field, social work, health care administration.
- Background in managing populations with complex medical and behavioral needs.
- Experience in community health setting or public health.
- Proven knowledge of Medicaid population.
- Experience in documenting in an electronic health record.
- Ability to organize and manage a caseload and assist with patient navigation.
- Full COVID 19 Vaccination is required.
- Ability to work independently and collaboratively to achieve goals.
- Highly self-motivated and organized.
- Proficiency with computer skills (i.e., Microsoft word, Excell and Access, PowerPoint, Web-based applications.)
- Demonstrated ability to work both independently and as an effective team member.
- Demonstrated experience working with diverse populations.
- Exercise sounds judgement and decision making.
- Excellent interpersonal skills.
- Excellent verbal and writer communication skills
- Bilingual-Spanish speaking a strong asset, although not required.
- High School Diploma/ GED
- Bachelor’s Degree (Preferred) License/ Certification
- Driver’s License
- CHW Course Completion
