The Enhanced Care Management (ECM) Clinician is responsible for member coordination, improving health outcomes, enhancing satisfaction, and reducing unnecessary healthcare utilization. ECM is a comprehensive, patient-centered approach to healthcare that aims to improve outcomes for high-need populations. This involves the coordination of medical, behavioral, and social services to address the full range of patient needs, focusing on those with complex and chronic conditions. ECM programs are designed to optimize care delivery, reduce unnecessary hospitalizations, and improve the overall quality of life for. The intention of our services is to help people who have not been able to get the needed help in terms of Housing, Mental Health, Substance Abuse treatment, and linkages to community resources to help minimize recidivism.
The role of The ECM Clinical Consultant provides clinical oversight, consultation, and quality assurance across multiple Enhanced Care Management (ECM) contracts spanning various Managed Care Plans and counties. This role is non-direct service and does not carry an individual caseload. The Clinical Consultant ensures clinical integrity, compliance, and quality of ECM services by reviewing and approving assessments and care plans, offering clinical guidance to staff, and supporting treatment-related decision-making.
The ECM Clinical Consultant plays a key role in ensuring services align with CalAIM/ECM requirements, evidence-based practices, and trauma-informed care principles, particularly for high-acuity populations, including justice-involved individuals.
This position is primarily office-based and telehealth-driven, with occasional travel required to support program implementation, training, and stakeholder engagement.
- Master’s or Doctoral degree in behavioral health, public health, or related human services field
- Active, unrestricted licensure in California:
- LCSW, LMFT, LPCC, PsyD, or PhD
- Minimum 5 years of clinical experience, including:
- Behavioral health (mental health and/or substance use)
- High-acuity or underserved populations (e.g., homeless, justice-involved)
- At least 2 years of experience in care management, population health, or ECM-related services
- Strong experience in:
- Clinical documentation review
- Treatment planning
- Crisis intervention
- Valid and unexpired driver’s license
Nice to Have
~1 min read- Previous Clinical experience working with individuals recently released from jail or prison a plus. Previous Supervisory experience.
- Experience providing services inside correctional facilities
- Knowledge of reentry systems, benefits navigation, and housing resources
- Experience with ECM/CalAIM programs and Managed Care Plans
- Experience working with justice-involved or reentry populations
- Previous supervisory or clinical oversight experience
- Knowledge of housing resources, benefits navigation, and community-based systems
- Comfortable working in both secure custodial environments and community-based field settings
- Required to have car insurance, a vehicle, and a valid driver’s license
- Bilingual, fluent Spanish/English oral and written skills are highly desirable.
- Required: Preferred: Licensed with the California Board of Behavioral Sciences (BBS) (e.g., ASW, AMFT, LCSW, LMFT)
- Preferred: Community Health Worker certification through California’s HCAI Department.

- Excellent interpersonal, communication, and facilitation skills, with the ability to effectively engage diverse stakeholders across community and organizational settings
- Expert knowledge of clinical practices, care coordination, and patient-centered care
- Strong analytical skills with the ability to interpret clinical and service utilization data and translate findings into actionable strategies
- Advanced understanding of medical necessity criteria, ECM/CalAIM requirements, behavioral health systems, and regulatory frameworks
- Strong clinical judgment, assessment, and problem-solving abilities
- Knowledge of trauma-informed care, reentry services, and working with justice-involved and high-acuity populations
- Ability to work independently while supporting multiple programs, contracts, and interdisciplinary teams
- Strong collaboration skills across counties, providers, and multidisciplinary teams
- Cultural competency and ability to work effectively with diverse and underserved populations
- Knowledge of contract parameters, deliverables, and compliance with local, state, and federal regulations
- Ability to support quality assurance efforts, including identifying gaps, recommending improvements, and ensuring program compliance
- Ability to manage multiple priorities, adapt to changing demands, and implement effective solutions
- Ability to clearly communicate concepts, prepare reports, and develop policies and procedures
- Proficiency in Electronic Health Records (EHR) systems and Microsoft Office Suite (Word, Excel, Outlook, etc.)
- Understanding and ability to apply organizational policies, procedures, and compliance standards
