Responsibilities
~1 min read- →Conducting pre-enrollment assessments for prospective enrollees utilizing clinical nursing expertise and critical thinking skills including obtaining preventive health service data.
- →Completes UAS - New York Assessment instrument to determine continuing eligibility and / or identify any needed changes to Care Plan.
- →Utilizes a skillful approach for assessing, coordinating, and evaluating consumer needs and will need to develop a prospective plan of care.
- →Communicates with the Care Team and participates in interdisciplinary team meetings.
- →Assessments will be conducted in Westchester. Reliable transportation required.
- →Responsible for environmental assessments.
- →Completes semi-annual assessments as requested
- →Maintains flexibility in schedule and responds to unexpected emergencies and changes in workload to fulfill responsibilities.
- →Conducts chart audits as requested if time permitting
Requirements
