ABOUT THE ROLE
We are looking for a compassionate, commercially aware Nurse Liaison to join our growing team. This is a varied, office and field-based role where you will combine clinical expertise with client relationship management and service development.
You will be responsible for assessing new clients, developing tailored care plans, and ensuring a smooth onboarding journey, while maintaining high standards of clinical care, compliance, and client satisfaction.
ABOUT YOU
You will be a Registered Nurse with active NMC registration and strong experience in clinical assessment and care planning. The successful candidate will demonstrate commercial awareness and the ability to balance client needs with service delivery, alongside excellent interpersonal and communication skills. You will be highly organised, detail-oriented, and confident working independently in a field-based role, with a strong understanding of CQC regulations and compliance standards.
KEY RESPONSIBILITIES
Client Onboarding & Assessment
- Respond to clinical enquiries, offering professional advice and support to prospective clients.
- Conduct in-person clinical assessments within clients’ homes to evaluate care needs.
- Develop and implement personalised care plans within 48 hours of initial assessment.
- Present and promote Draycott’s services, clearly articulating how they enhance client wellbeing and quality of life.
- Provide transparent pricing guidance and recommend cost-effective care solutions tailored to individual budgets.
- Complete initial risk assessments and ensure safe, effective onboarding of all clients.
- Liaise with Care Coordinators, the Care Team, and the Care Quality Liaison team to ensure the right skills and capacity are in place.
- Ensure a smooth and safe transition for clients into their care packages.
Client Relationship Management & Growth
- Build and maintain strong relationships with clients and their families.
- Conduct follow-up visits to ensure satisfaction and ongoing service quality.
- Gather feedback and support the generation of online reviews.
- Manage a caseload of clients, carrying out regular reviews and reassessments.
- Meet referral and conversion targets, supporting business growth while ensuring ethical, client-centred care.
Ongoing Clinical Duties
- Carry out regular home visits for new and existing clients with changing or complex needs.
- Update care plans and risk assessments in line with evolving requirements.
- Escalate safeguarding concerns or urgent clinical issues appropriately.
- Work collaboratively with multidisciplinary professionals (GPs, Physiotherapists, Occupational Therapists, etc.).
- Promote health, well-being, and independence among clients.
- Ensure infection prevention and control guidelines are consistently followed.
Audits, Compliance & Risk Management
- Support the maintenance and continuous improvement of CQC standards.
- Ensure all care documentation meets regulatory and compliance requirements.
- Oversee medication management, including eMAR compliance.
- Investigate incidents and complaints, working with the Registered Manager to implement appropriate actions.
- Carry out audits as required and contribute to quality improvement initiatives.
- Monitor and respond to Early Warning Systems (EWS).
- Participate in the on-call rota to provide out-of-hours support (additional pay provided).
Field Team Support
- Conduct supervisions and appraisals for nurses in the field.
- Carry out spot checks and competency assessments.
- Lead medication error investigations with support from the wider team.
- Act as a role model, supporting training, compliance, and best practice across the Care Team.
