We're seeking a dedicated RN  or Master Social Worker to make a meaningful impact in patients' lives by coordinating safe, seamless transitions from hospital to home. In this rewarding role, you'll work directly with patients and their families to identify needs and arrange essential post-acute services including home health, durable medical equipment, and rehabilitation placements. We provide comprehensive 6-week orientation with experienced preceptors who will guide you through our systems including Windows 11, Cerner, CareAware, and Microsoft Teams—no prior experience with these platforms required.
