• →Provide rehabilitative care and skilled physical therapy services to patients in their homes
• →Develop and implement individualized treatment plans
• →Collaborate with physicians, nurses, occupational therapists, speech therapists, and caregivers
• →Educate patients and families to promote independence and improve quality of life
• →Complete timely clinical documentation using electronic medical records

• Active Physical Therapist license in New Jersey
• Graduation from an accredited Physical Therapy program
• Home health experience preferred but not required
• Strong clinical assessment and communication skills
