BAYADA Home Health Care is seeking an experienced Weekend Admissions Physical Therapist (PT) to join our Essex County team. This per diem opportunity is ideal for clinicians looking to supplement their income while enjoying the flexibility of a weekend schedule and the autonomy of home health practice.
In this specialized role, you'll focus on completing OASIS Start of Care (SOC) admissions for adult and geriatric clients, helping patients begin their home health journey with comprehensive evaluations and individualized care plans.

Responsibilities
~1 min read- →Complete OASIS Start of Care (SOC) admissions visits for adult and geriatric home health patients.
- →Conduct comprehensive physical therapy evaluations, including assessments of mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
- →Develop individualized treatment plans based on patient needs and physician orders.
- →Implement appropriate therapeutic interventions and recommendations to support recovery and independence.
- →Collaborate with nursing and interdisciplinary team members to coordinate patient care.
- →Educate patients, families, and caregivers on rehabilitation strategies and safe mobility techniques.
- →Continuously assess patient status and revise care plans as clinically appropriate.
- →Accurately document evaluations and services using electronic medical records and tablet technology.
Requirements
~1 min read- Current New Jersey Physical Therapist (PT) license.
- Minimum of one year of recent experience as a licensed Physical Therapist.
- Home health experience required.
- OASIS experience required.
- HCHB experience strongly preferred.
- Excellent assessment, communication, and time management skills.
- Ability to work independently while providing exceptional patient care.
