Position Summary
The Primary Care Nurse serves as a central point of clinical coordination and a key day-to-day support resource for the care team. This role is responsible for managing clinical workflows while also supporting team operations, coverage needs, and real-time problem solving.
The Primary Care Nurse acts as a go-to resource for team questions and urgent issues, helps ensure efficient and consistent workflows, and contributes to training, onboarding, and team performance. Success in this role requires strong clinical judgment, the ability to multitask in a fast-paced environment, and a proactive, team-oriented approach.
Key Responsibilities
Clinical Coordination
- Serve as primary point of contact for patients, families, and partner facilities; triage clinical needs and ensure timely follow-through
- Manage provider-directed workflows including labs, orders, referrals, and care transitions
- Ensure accurate and timely communication with providers and care partners
- Support daily coverage needs and scheduling gaps to maintain continuity of care
- Act as a go-to resource for team questions and urgent issues
- Assist with coordinating workflows to ensure tasks are completed efficiently and consistently
- Assist with onboarding new team members, including orientation and workflow training
- Provide ongoing peer support, guidance, and reinforcement of best practices
- Help promote consistency in processes and clinical workflows
- Support tracking and follow-up of team KPIs and performance metrics
- Identify workflow gaps and contribute to process improvement efforts
- Help reinforce accountability for task completion and quality standards
- Communicate clearly and professionally with providers, nurses, and support staff
- Demonstrate the ability to handle difficult conversations with professionalism and accountability
- Foster a positive, collaborative, and team-oriented work environment
Education & Licensure:
- Active LPN/RN license required
- Minimum 2–3 years of nursing experience in primary care, home health, or care coordination preferred
- Experience working in a fast-paced, high-volume environment preferred
- Strong clinical judgment and problem-solving ability
- Excellent multitasking and organizational skills
- Effective communication and interpersonal skills
- Ability to work independently as a self-starter while contributing as a team player

The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Colorado typically falls between LPN $28-32 hourly / RN $39-44 hourly with the potential for performance-based bonuses and other benefits. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
