Level of Care Nurse Specialist

As a Level of Care Nurse Specialist, you'll use your clinical knowledge, assessment skills, and sound judgment to determine whether prospective participants meet the nursing home level of care requirements for enrollment in the Program of All-Inclusive Care for the Elderly (PACE). Through in-home assessments, collaboration with state agencies, and partnership with our Interdisciplinary Team (IDT), you'll help ensure seniors receive the care and support they need.

This position is ideal for someone who is compassionate, organized, and confident working independently while collaborating with others. You'll be part of a supportive, participant-centered team where clinical excellence, communication, and continuous improvement are valued.

First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

• →Conduct comprehensive in-home Level of Care assessments for prospective PACE participants.
• →Determine eligibility based on federal and state nursing facility level of care requirements.
• →Complete accurate, timely, and audit-ready documentation in state systems and the electronic medical record.
• →Coordinate with state agencies to complete eligibility determinations.
• →Maintain all required state certifications and annual training.
• →Collaborate with the Interdisciplinary Team to communicate assessment findings and support participant enrollment.
• →Prepare concise clinical summaries highlighting participant needs, risks, and recommendations.
• →Identify changes in condition, safety concerns, or barriers to care and promptly communicate findings to the care team.
• →Assist with the development of initial care plans based on assessment findings.
• →Educate participants and families about PACE services and level of care requirements.
• →Monitor reassessment timelines and ensure all required deadlines are met.
• →Participate in quality improvement initiatives that enhance participant outcomes and assessment consistency.
• →Maintain confidentiality and comply with all HIPAA, regulatory, and organizational standards.
• →Provide hands-on support to the clinic when needed and as level of care schedule allows.

• Clinical proficiency

• Graduate of an accredited nursing program (LPN/LVN, Diploma, ADN, or BSN)
• Current unrestricted nursing license in the applicable state.
• Two years of nursing or case management experience preferred.
• Experience working with the frail or elderly population is preferred, but we welcome nurses who are eager to learn through our training and mentorship programs.
• Knowledge of Medicare, Medicaid, or PACE regulations is a plus.

• Must be able to move intermittently throughout the workday.
• Must be able to stand, walk, bend, reach, kneel, and assist participants during assessments as needed.
• Must be able to lift, push, pull, and move equipment or materials as required.
• Must be able to travel to participant homes throughout the service area.
• Must be able to work under deadlines while providing compassionate care to frail, elderly participants.
