Help older adults stay safe, independent, and at home, and get a $5,000 sign on BONUS with eligibility to earn additional quarterly bonuses!

This is not traditional physical therapy. It is the opportunity to use your clinical judgment where it matters most: helping older adults maintain mobility and continue to live in the homes know and love.

Join an interdisciplinary PACE team built around access, outcomes, and meeting needs, not visit restrictions, productivity, or limited access to basic DME needs. This role is based at the PACE center and includes home visits, allowing you to assess and treat participants in both a collaborative clinical setting and the environments where they live.

  • Clinical decisions driven by need: Recommend the right intervention at the right time, with care planning centered on participant goals and safety rather than traditional insurance visit constraints.
  • Real continuity of care: Build trusted relationships and follow participants over time, adjusting the plan as health, function, environment, and priorities change.
  • True interdisciplinary practice: Work side by side with primary care, nursing, occupational therapy, social work, nutrition, transportation, home care, and other partners.
  • Impact beyond the treatment space: Influence mobility, fall prevention, caregiver support, equipment, home safety, and successful transitions across settings.
  • Meaningful complexity: Apply advanced clinical reasoning to medically and socially complex situations where your expertise can prevent decline, hospitalization, or facility placement.
