What We Offer

Provide guidance to the team members in regular transdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitation of problem solv­ing sessions; and delivery of team support.
Assess daily staffing needs and implements re-distribution of resources based on census to maximize patient care.
Periodically audit selected clinical records for documentation concerning HHA supervision, visits/matching orders, and other areas related to regulatory compliance and enter findings in audit database. Hold staff accountable to standards.
Coordinate transdisciplinary team meetings and supervise the planning, coordination, and delivery of the transdisciplinary team interventions.
Conduct appropriate supervisory visits to allow for clinical observation of performance of assigned team members in accordance with agency policy, accreditation, and regulatory standards.
  • BSN in Nursing or Master’s degree in social work from a school accredited by the Council on Social Work Education
  • Minimum of two years of nursing or social work practice experience ideally in a home health, hospice or community health setting within the past five years.
  • Five years supervisory experience or relevant progressive management experience preferred
  • Current license as a registered professional nurse in the State of California or current Licensed Clinical Social Work (L.C.S.W.) certification in the State of California
  • Public Health Nursing certification highly desirable.
  • HPNA certification preferred.

Pay Range:
$135K - $162K annually
