About the Role
~1 min readWe are seeking Board Certified/Board Eligible Internal Medicine physicians to provide on-site Hospital Medicine (Hospitalist) services to Veterans at the Fargo VA Health Care System in Fargo, North Dakota. In this role, you will deliver comprehensive inpatient care to hospitalized Veterans, serving as the coordinating physician across the full episode of hospital care — from admission through discharge — in a facility that operates 24/7, 365 days a year. Approximately 95% of your time will be devoted to direct patient care, with the remaining 5% dedicated to administrative and quality improvement activities.
- Hospitalist coverage is provided 24/7 in two 12-hour shifts: Day shift 7:00 AM – 7:00 PM and Night shift 7:00 PM – 7:00 AM (local time).
- Full-time schedule is seven (7) 12-hour shifts per two-week payroll period (approximately 2,184 hours per year), including federal holidays.
- A minimum of two (2) Board Certified/Board Eligible Hospital Medicine physicians are on site daily.
Responsibilities
~2 min read- →Admit hospitalized medical patients and coordinate all aspects of their care, including history and physical examinations and appropriate placement within the hospital.
- →Evaluate and treat patients, including ordering and interpreting diagnostic studies (laboratory, diagnostic imaging, and electrocardiographic examinations) and administering medications within the practice of hospital medicine.
- →Provide care in critical/acute care settings; assess, stabilize, and determine disposition of inpatients with emergent conditions, including running code teams and rapid response teams.
- →Discuss end-of-life preferences and advance care planning with patients and families.
- →Coordinate consultations with sub-specialists and communicate with referring clinicians on appropriate work-up and course of treatment.
- →Review all inpatient admissions recommended by Physician Assistant or Nurse Practitioner providers, with person-to-person hand-offs for every admission.
- →Lead timely, proactive discharge planning, including discharge education, follow-up instructions coordinated with the next care setting, and coordination of care with the primary care physician.
- →Provide perioperative care as appropriate.
- →Initiate social work referrals for identified homeless Veterans and patients without primary care providers.
- →Perform core hospital medicine procedures within scope of privileges, which may include: abscess incision and drainage, thoracentesis, paracentesis, lumbar puncture, nasogastric tube insertion, gastric tube reinsertion, transurethral catheter placement, joint aspiration and injection (except ankle and hip), lateral epicondyle injection, removal of lines and tubes, fecal occult blood testing, and all ACLS procedures.
- →Document all care in the VA electronic health record (CPRS), selecting appropriate E&M and procedure codes, with documentation and coding completed by the end of each patient care encounter.
- →Participate in quality improvement activities, staff meetings, and QA/QI documentation as required by the facility Chief of Service or Chief of Staff.
Requirements
~2 min read- MD or DO with Board Certification or Board Eligibility in Internal Medicine (ABIM).
- Current, full, and unrestricted license to practice medicine in any U.S. State, Territory, Commonwealth, or the District of Columbia. A North Dakota medical license is preferred. Candidates who have ever had a license restricted, suspended, revoked, voluntarily surrendered pending action, or denied upon application cannot be considered.
- Current VHA-approved Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification (or equivalency).
- Experience providing the scope of hospital medicine care described above.
- Current DEA registration and National Provider Identifier (NPI).
- Ability to speak, understand, read, and write English fluently.
- Current CME in compliance with national/medical association minimum standards.
- Must successfully complete VA credentialing and privileging (VetPro) in accordance with VHA Directives 1100.20 and 1100.21 prior to providing services.
- Must provide proof of immunizations/screenings current within the past year, including TB testing (TST or IGRA), MMR and varicella immunity, Tdap, annual influenza vaccination, and Hepatitis B series with bloodborne pathogen training.
- Must complete all VA-mandated training (e.g., EHR/CPRS and TMS courses) as required.
- Must not appear on the HHS OIG List of Excluded Individuals/Entities.
- Verifiable evidence of all education and training, including explanation of any gaps in educational history.
