Home Care Transition Nurse - Farmingdale

The Home Care Transition Nurse is responsible for assessing, planning, and developing referrals.

• 5 x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

• Additional compensation for visits over your caseload

This position will be on site at St. Charles Hospital

• Registered Nurse with current licensure in NYS 
• 2 or more years of home care experience preferred
• Customer Service and Marketing skills required
