Located on a beautiful 10-acre campus in the heart of Scottsdale, Devereux Arizona’s 24-hour Residential Treatment Center (RTC) has the capacity to serve 52 children. The RTC is a safe, supportive environment that provides various services to youth, from the ages of 7 to 17, who are experiencing emotional, behavioral, or substance abuse challenges. The Campus features dormitories, a therapeutic K-12 day school, food services, medical/nursing, and therapy services.
Reporting to the Clinical Program Manager, the Care Coordinator works cooperatively with clinical, operations, medical, and administrative staff on the overall coordination of individual care at the RTC. The Care Coordinator participates in family engagement/outreach, admission/intake processes, discharge and treatment planning, and documentation of all care coordination activities accordingly. The position is responsible for keeping the treatment team informed of assigned cases through team meetings, tracking and communicating due dates of reports, and responding to care management inquiries for clinical staff. Other job duties include: responding to requests for visitations/passes, assisting with admissions processes, clinical documentation, and completing required reports for both internal and external purposes.

- Drug test within 24hrs of verbally accepting offer w/ negative results
- Must be available to attend 2 weeks of required orientation training (paid, Mon-Fri 8:30am-5:00pm) plus additional 2-3 weeks on the job training
- Valid Arizona Driver's License & pass DMV 36-month history check
- Pre-employment Physical and Job Demand Assessment (if applicable)
- Current AZ Fingerprint Clearance Card with no restrictions (or eligibility to obtain with no restrictions). Arizona Revised Statutes Notice (ARS 36-411 Residential care institutions; nursing care institutions; home health agencies; fingerprinting requirements; exemptions; definitions). If a person's employment record contains a six-month or longer time frame during which the person was not employed by any employer, a completed application with a new set of fingerprints shall be submitted to the department of public safety.
