Responsibilities
~1 min read- Proactively partner with organizational leaders to understand operational priorities, assess workforce development needs, and identify competency and performance gaps.
- Conduct training needs assessments and recommend effective learning solutions, collaborating with appropriate departments when challenges extend beyond training.
- Partner with the Senior Director to develop and execute learning strategies, establish program objectives, KPIs, and appropriate ROI measures, and evaluate organizational impact.
- Conduct market research and benchmarking to identify emerging learning approaches, industry trends, and opportunities for improvement.
- Design, develop, and facilitate engaging training programs for workforce and leaders organization wide.
- Evaluate existing learning content in collaboration with subject matter experts to ensure accuracy, relevance, and effectiveness, identifying opportunities to update, redesign, consolidate, or retire materials.
- Develop practical learning solutions, including instructor-led training, interactive workshops, digital learning modules, facilitator guides, job aids, and toolkits.
- Develop train-the-trainer programs to strengthen facilitation capabilities and ensure consistent training delivery across departments.
- Leverage Learning Management Systems (LMS), digital learning platforms, and emerging technologies to improve learning accessibility, engagement, delivery, tracking, and reporting.
- Design and facilitate leadership development programs focused on communication, coaching, accountability, performance management, employee engagement, and change leadership.
- Partner with leaders to identify development opportunities for new, emerging, and experienced leaders.
- Support onboarding, professional development, career pathways, internal mobility, and succession planning through targeted learning initiatives.
- Collaborate with leaders to reinforce learning in the workplace, strengthen workforce competencies, support ongoing professional growth, and drive change management initiatives through targeted learning solutions.
- Partner with Clinical Trainers, Health Home Care Management, Operations, and other departmental leaders to identify learning needs and develop role-specific training programs.
- Design, coordinate, and facilitate training initiatives that strengthen Health Home care management competencies, care coordination, patient engagement, patient experience, and service delivery.
- Collaborate with subject matter experts to develop and improve learning programs covering care management workflows, patient-centered care, documentation, compliance, quality standards, and escalation processes.
- Support the development and continuous improvement of training for Integrated Care Coordinators (ICCs), care management teams, and other patient-facing and non-patient-facing members of the workforce.
- Promote consistent operational practices, effective communication, professionalism, accountability, and quality patient care through practical learning solutions.
- Evaluate training effectiveness through learner feedback, knowledge assessments, competency development, behavioral change, and operational performance indicators.
- Analyze training data to measure progress, identify trends, and recommend improvements to learning programs.
- Present training outcomes, insights, and recommendations to senior leadership, demonstrating the connection between learning initiatives and organizational performance and advising on strategic direction rather than solely executing on request.
- Continuously assess learning processes, delivery methods, and resources to strengthen program effectiveness and the workforce learning experience.
Requirements
~2 min read- Bachelor's degree in Human Resources, Organizational Development, Education, Business, or a related field.
- Minimum of 8 years of progressively responsible experience in Learning & Development, Organizational Development, or a related field.
- Demonstrated experience conducting learning needs assessments, designing and facilitating training programs, and evaluating learning content for workforce and leaders at various organizational levels.
- Proven ability to independently manage training and development initiatives from needs assessment and program design through implementation, evaluation, and continuous improvement.
- Strong facilitation, presentation, instructional design, and project management skills, with exceptional attention to detail.
- Ability to proactively build relationships, influence stakeholders, and collaborate effectively with leaders across departments and organizational levels.
- Strong analytical and problem-solving skills, with the ability to use data, performance metrics, and feedback to assess learning effectiveness and recommend improvements.
- Excellent communication and interpersonal skills, with the ability to manage competing priorities and adapt to evolving organizational needs.
- Experience in healthcare, Health Home care management, or multi-site organizations; a clinical background is not required.
- Experience designing or facilitating training related to care coordination, Health Home care management, patient engagement, patient experience, or healthcare operations.
- Experience developing and implementing leadership and management development programs.
- Experience establishing training KPIs, measuring program outcomes, and evaluating learning effectiveness and ROI.
- Familiarity with healthcare operations, regulatory and compliance training, and workforce development practices.

- Microsoft Office Suite, including Excel, PowerPoint, and reporting tools.
- Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems, such as Epic, eClinicalWorks, Athenahealth, or Oracle Health/Cerner.
- Workforce management and operational systems, including UKG and Twilio Flex.
