Responsibilities
~1 min read- →Patient Evaluations: Conduct comprehensive assessments of patients’ health status, including physical examinations, medical history reviews, and medication reconciliations during home visits.
- →Diagnosis and Treatment: Diagnose medical conditions, develop and implement individualized treatment plans, and prescribe medications as needed.
- →Care Coordination: Collaborate with physicians, specialists, and other healthcare providers to ensure continuity of care and comprehensive support for patients.
- →Patient Education: Provide education to patients and their families regarding health management, lifestyle modifications, disease prevention, and medication adherence.
- →Documentation: Maintain accurate and thorough documentation of all patient interactions, including assessments, treatments, and follow-up care in electronic health records.
- →Follow-Up Care: Schedule and conduct follow-up visits to monitor patient progress and adjust treatment plans as necessary.
- →Emergency Response: Assess and manage urgent medical issues during home visits, providing referrals or interventions as needed.
- →Community Engagement: Participate in outreach efforts to promote health services and educate the community on available resources.
Requirements
~1 min read- Master's Degree in a Nurse Practitioner Program: Family or Adult Geriatrics
- Current New York State License as a Nurse Practitioner.
- National Board Certification require
- American Association of Nurse Practitioners (AANP)
- American Nurses Credentialing Center (ANCC)
- Oncology Nursing Certification Corporation (ONCC)
- Bilingual (Spanish Speaking) is a plus
- EMR/EHR experience a plus

- Primarily operates in patients’ homes, which may vary in living conditions.
- Requires travel within a designated geographic area, with a flexible schedule to accommodate patient needs, including possible evenings and weekends.
- May involve working collaboratively with a multidisciplinary team.
Salary: $135,000-$200,000
