As a part of the Goldbelt Glacier Team, you will be supporting the Naval Logistics Command with medical services for active-duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries designated by the Government. As a HealthCare Worker (HCW), you will be assigned to either a Military Treatment Facility (MTF) or other Federal medical treatment facilities in Japan.
Responsibilities
~1 min read- Schedule will consist of 12-14 hour shifts. A maximum of four consecutive shifts may be permitted subject to Government approval. All HCWs are required to work across all shift variations at various times—including day shifts, night shifts, weekday duty, and weekend duty.
Draft schedules are created approximately four weeks in advance and finalized approximately two weeks before the start of the period. The schedule is managed by schedulers appointed by flight leadership. Any changes to the finalized schedule must be approved IAW OI 46-06. Schedules are posted in two locations: a physical in-office posting and through the Signal group chat.
All leave and time-off requests must be submitted IAW the OI and are subject to approval based on mission and staffing needs.
HCW may request up to fourteen (14) consecutive days off. Any request for an approved absence exceeding fourteen (14) consecutive days requires additional approval from the Squadron Commander.
- Work schedule.
- The HCW is required to perform seven (7) shifts of hours per pay period. The HCW may be required to work more than 12 hours but no greater than 14 hours in a single shift, as determined by the supervisor or designee based on the needs of the MTF. Pay periods are two week intervals. The daytime shift schedule is 0545-1800. The night shift is 1745-0600.
- The HCW will be required to augment clinical nursing support at the direction of the Flight Commander/scheduler during the absence of other team nurses.
- The contractor’s schedule will be based on a "Panama" shift schedule. An example Panama schedule looks something like the following:
- Work: Monday, Tuesday
- Off: Wednesday, Thursday
- Work: Friday, Saturday, Sunday
- Off: Monday, Tuesday
- Work: Wednesday, Thursday
- Off: Friday, Saturday, Sunday
- (The cycle then repeats from the top again.)
- The government will provide physical paper sign-in sheets at the ward/unit/clinic where HCWs will record their arrival and departure times. The government reserves the right to verify hours worked by the contract employee by implementing a sign-in/sign-out procedure or by using a time clock at a site other than the assigned ward/unit/clinic. DHMRSi will be mandatory for completion of all contractors.
Submission. The completed timesheet, signed by FRED as monthly confirmation, must be submitted by FRED to the COR within three business days of the following month.

For any scheduled shift missed due to an approved absence, the HCW shall not record arrival or departure times. The scheduled shift line on the physical paper sign-in sheet shall be annotated as "APPROVED ABSENCE – NON-BILLABLE" by the Ward/Unit Supervisor or designated Government representative.

- Invoice Reconciliation Standard:
- The physical sign-in sheet constitutes the sole Government verification record for billing. The Prime Contractor shall ensure that all approved absence hours are deducted from monthly billing submissions. Invoiced labor hours must reflect only actual hours physically worked; zero ($0.00) charges shall be billed for any approved absence periods.
On-Call. Reserved.
As part of the HCW’s normal hour of duty, service will be required on the day of observance of Federal holidays and Non-Duty Days/Closures. Refer to OI 46-06.
- Telework/Remote work. Telework/Remote work is NOT authorized for any positions on this Task Order. Contractor personnel must be present in MTF to provide service or absence if they are unable to report to work.
Non-Duty Days/Closures. Due to the 24-hour operation of the Labor and Delivery Unit, if the commander determines the base is closed due to snow, hurricane, or acts of God for non-essential personnel, the unit remains open and the HCW is still expected to show for work unless physically unable to travel to the assigned duty location. If unable to make it to work, the HCW shall annotate their time sheet as such and notify the supervisor and charge nurse that they are unable to report to work. Such occurrences are not billable to the government. The contractor shall only invoice for actual hours worked. Under no circumstance will the government be liable for hours not worked as a result of base closure or excusal from duty by the commander, regardless of the reason (emergency or otherwise).
- MTF Closure for Non-Clinical Activities:
- The MTF may occasionally close for patient care to conduct command-sponsored activities, such as training or other official events. These days are considered regular workdays for contract Health Care Workers (HCWs). Contractor personnel are required to participate in these activities as directed. If a contract HCW does not attend, they must request and receive approval for an absence in accordance with (IAW) section 9.3.
MTF Closures for Pacific Air Forces (PACAF) Family Days or Wing Down Days.
IAW IDIQ PWS paragraph C.3.2.13, the COR or designee shall notify the contractor/HCW fourteen (14) calendar days prior the family day if MTF facility will be closed. If the MTF facility is open for patient care, HCW should report to work as scheduled. If the MTF facility is closed for patient care, HCW shall not report to the MTF. Alternate work sites are not approved. If the MTF facility is closed, these days are considered non-billable to the Government. Historically there are ten (10) PACAF Family Days annually.
Absences:
Preplanned Excused: A Preplanned Excused Absence is an authorized, pre-approved period of time during which the HCW is permitted to be absent from a scheduled shift or required assignment without incurring contractual penalty or disciplinary action.
Submission Timeline: Follow the guidance in OI 46-06.
Unplanned / Emergent / Excused Absences
Non-prearranged absences resulting from illness, medical emergencies, or unforeseen events that satisfy established notification and contractor substantiation requirements
Notification Requirements: The Healthcare Worker (HCW) or Contractor representative shall notify the FRED and Charge Nurse at least two (2) hours prior to the scheduled shift start time. In cases of sudden acute illness or emergency where two hours notice is impossible, notification shall be provided immediately upon occurrence prior to the shift start.
Substantiation & Government Verification: For an absence of three (3) or more consecutive scheduled shifts, the Contractor—as the employing entity—shall obtain and maintain underlying medical substantiation in accordance with applicable employment laws and privacy standards. The Contractor shall provide formal written certification of the excused absence to the Contracting Officer’s Representative (COR) upon request.
Request Offs (RO): HCWs may submit ROs IAW OI 46-06. RO days represent non-work preference requests and are not guaranteed. A maximum of two (2) days off (scheduled days off or Request Off days) may be taken immediately before, after, or surrounding an approved excused absence period, mission permitting.
Unexcused: Any absence lacking timely notification, required documentation, or supervisor pre-authorization is classified as unexcused/unauthorized. All unexcused absences shall result in formal escalation.
- Scheduled and Unscheduled Absences.
- Contractor personnel are allowed to take up to twenty (20) workdays of approved absence annually. Approved absence is classified as both annual, vacation or sick leave. Approved absence shall be taken in one (1) day increments and shall not exceed ten (10) consecutive workdays. All approved absences are considered non-billable days. Timing and details shall be coordinated with the COR. Scheduled absences shall be scheduled at least thirty (30) calendar days in advance and mutually agreed upon by the COR and individual. Unscheduled absences shall be called into the COR by the contract provider within the first two (2) hours of each duty day that HCW is unable to report to work.
Duties:
The Healthcare Worker (HCW) will participate in an initial one week facility and unit-specific orientation (Monday through Friday, 0600–1400, with flexible scheduling as required to complete core clinical exposures including one delivery, one discharge, one outpatient intake, and one triage event, when possible per patient census). The designated unit orientation checklist and competency assessments must be completed and validated by the unit Charge Nurse / Clinical Preceptor within this one -week timeframe.
During the initial 30-day performance assessment period (comprising the one-week orientation and the subsequent two weeks of independent practice), the HCW's performance will be monitored for compliance with PWS standards, clinical competency, and patient safety protocols. Any failure to meet contract performance standards will be documented and forwarded through the Contracting Officer’s Representative (COR) to the Contractor for corrective action or replacement.
Coordinates patient care through a continuum and facilitates the achievement of high quality, safe and cost-effective care.
Participates in the orientation and training of newly assigned personnel, as appropriate.
Ensures compliance with standards of care and practice in accordance with all established Unit Operating Instructions (which follow national guidance from AWHONN, American College of Obstetricians and Gynecologists (ACOG) and the American Academy of Pediatrics (AAP), Medical Group Instructions, and Air Force Instructions used in the medical treatment facility. Provides care within ethical and legal boundaries.
Provides appropriate patient assessment skills.
Accountable for making patient care assignments based on the scope of practice and skill level utilizing the nursing process as a basis for professional practice.
Sets up, operates, and monitors specialized medical equipment such as IV infusion pumps, PCA pumps, vital sign machines, electronic fetal monitoring, cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, ultrasound, emergency transport and other unit-specific equipment.
Recognizes adverse signs and symptoms and reacts quickly in emergency situations.
Makes referral appointments and arranges specialty care, as applicable.
Completes orientation and competency verification programs in accordance with unit guidelines.
Serves and participates in committees, functions and other meetings as directed. Provides relevant and timely information to these groups, and assists with decision making and process improvement. Participates in customer service initiatives and medical readiness activities designed to enhance health services.
Performs sterile vaginal examinations, speculum examinations, and diagnostic procedures via electronic fetal monitoring and tocometer.
Prepares patients for surgery, ultrasound, amniocentesis, or other prescribed medical procedures.
Provides critical care and evaluation of patients.
Develops nursing care plans, performs care as outlined in clinical pathways, and standing order sets.
Provides and documents individual and family education.
Prepares sterile supplies, non-sterile supplies, and ensures required equipment is present and ready for use.
Adjusts equipment as required. Reports all problems to the Charge Nurse and works with maintenance service technicians to address problems.
Check the operating parameters of the equipment each time before the equipment is used.
Develop and monitor quality improvement programs with peer reviews, staff education, chart reviews, and statistics.
Performs telephone consults, triage, and other necessary administrative tasks as required,
Maintain knowledge of how to provide nursing care from a multi-disciplinary continuum, from normal physiological changes to a wide variety of disease processes that may complicate the ante-partum, intra-partum or postpartum stages of pregnancy, the immediate care of the newborn and have a varied knowledge in women’s health issues and the physiological changes in the normal life cycle.
Maintain knowledge of the variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required.
Maintain knowledge of the administrative requirements for proper documentation of patient’s condition including disease progress, acknowledgement of teaching, and follow up care.
Maintain knowledge of computer operations and proficiency in the use of basic word processing, data entry and automated medical records.
Maintain knowledge of effective communication and collaboration with the health care team, both in oral and written form.
Maintain knowledge of synthesizing data from a variety of sources to make appropriate clinical decisions.
Documentation. The provider shall prepare all medical documentation to meet or exceed the standards required under AFI 46-101 Nursing Services and Operations, Association of Women’s Health, Obstetric and Neonatal Nurses (AWHONN), applicable Air Force Instructions and the Joint Commission guidelines. Only approved abbreviations shall be used for documentation of care in the health care record. The Department of Defense’s electronic medical record systems, MHS Genesis shall be used for all patient encounters.
The general scope of work under this contract includes performing services in the following patient care areas: Antepartum Labor & Delivery Postpartum Neonatal via Rooming-In or Nursery Obstetric Triage
The provider shall prepare all medical documentation to meet or exceed the standards required under AFI 46-101 Nursing Services and Operations, Association of Women’s Health, Obstetric and Neonatal Nurses (AWHONN), applicable Air Force Instructions and the Joint Commission guidelines. Only approved abbreviations shall be used for documentation of care in the health care record. The Department of Defense’s electronic medical record systems, MHS Genesis shall be used for all patient encounters.
Provide nursing care to routine and high-risk Obstetrical patients including newborns during the transition period within assigned inpatient unit based on relevant specialty care standards.
Establish priorities of patient care based on triage, nursing diagnosis, essential patient needs and outcomes.
Collaborate with multi-disciplinary team in assessing the effectiveness of existing therapies and interventions for patients with illnesses, their families and significant others.
Apply understanding and operation of specialty equipment. Maintain a safe patient care environment. Monitor and troubleshoot equipment assuring appropriate patient response. Provide intervention when indicated.
Ensure upkeep, perform checks and maintain appropriate logs of medical equipment.
Administer medications and IV therapy employing knowledge of pharmacological agents and blood/blood components, their indications, routes of administration, expected actions and potential side effects, as related to specific patient population served. Manage complex central and peripheral intravenous lines/infusions.
Provide recovery care for post-anesthesia patients following C-Sections, cerclage, tubal ligations, and spontaneous pregnancy loss.
Serve as a patient advocate and maintain patient’s right to confidentiality. Assist patients, families, and significant others in identifying limitations.
Provide telephone advice based on clinical knowledge and accepted standards. Assist as needed in accessing appropriate ambulatory/emergency care for beneficiaries.
Consult and work collaboratively with social work and other ancillary services at the medical center and in the community to aid patients in maintaining independence, dignity, and quality of life.
Evaluate effectiveness of own performance.
Contribute to health care system and support clinical environment resulting in quality services and positive patient outcomes.
Assist management and staff to develop and implement standards of care in keeping with unit and command’s strategic patient care goals.
Demonstrate clinical expertise and independent practice necessary to function as a resource for medical and nursing personnel and mentor to staff. Provide clinical consultation and participate in problem-solving efforts of nursing staff, medical staff and other disciplines.
Conduct and participate in patient bedside rounds with care team, patient care conferences and discharge planning.
Assess and address the learning needs of patients and families, as well as, cultural and spiritual needs, individualizing each plan of care to meet patients’ specific needs.
Participate in formal patient education and development of patient education materials.
Monitor patient responses to medications/procedures/treatments and provide timely, accurate, and concise documentation, manually and/or by use of computer.
Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events.
Assist in the development, application and monitoring of clinical pathways as directed by the government.
Assist with the development of programs that comply with all regulatory agencies and Joint Commission requirements with a focus on CI, professional practice, and patient care/teaching.
- Complete all MTF-specified orientation program(s), initial and annual training requirements, and comply with all MTF policies, procedures, productivity standards and instructions as provided by the MTF.
Requirements
~3 min read- Shall possess a bachelor’s degree in nursing from a college or university accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE), or a foreign equivalent evaluated by an accredited credentialing agency (e.g., Commission on Graduates of Foreign Nursing Schools (CGFNS) or TruMerit).
- Minimum of two (2) year experience as an inpatient labor and delivery nurse is required. One (1) year of experience with postpartum and newborn nursery is highly desired. All experiences should be within the last five (5) years. Have a work experience for Level 1 Labor and Delivery, American College of Obstetricians and Gynecologists (ACOG) for reference level of maternal care is highly recommended.
- Certifications:
- Advanced Life Support (ALS)
- Basic Life Support (BLS)
- Neonatal Resuscitation Program (NRP)
- Intermediate Fetal Heart Monitoring - One of the following is required: Association of Women’s Health Obstetrics and Neonatal Nurse (AWHONN), GE healthcare, or National Certification Corporation (NCC) test for Electronic Fetal Monitoring (EFM). The Government may approve an alternate course.
- Obtain and Defense Health Agency (DHA) competencies for labor and delivery nurses within 30-60 days, as well as certifications and competencies such as NMRTC Yokosuka nurse corps competencies, unit/departmental competencies, command orientation (e.g., IV, medications and blood administration) in accordance with the directorate nursing services and base-wide command policy not later than 60-90 days after commencement of services, including MTF observation of three completed IV sticks during this time.
- Possess and maintain a current unrestricted license to practice as a registered nurse in any one of the 50 States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands
- HCW shall be in good standing, and under no restrictions, with the state licensure board in any state in which a license is held or has been held within the last 10 years.
- HCW shall be in good standing and under no sanction or suspension by the Federal Government.
- Possess a Neonatal Resuscitation Provider (NRP) certifications. HCWs not currently in possession of current certification must acquire certification prior to initiating contract performance. Web based classes do not meet these standards.
- Able to read, write, speak, and understand the English language fluently and maintain good communication skills with patients and other health care personnel.
- Ability to pass all background/suitability checks.
- A valid driver’s license; ability to travel locally as needed.
- Must be a US Citizen
- A thorough understanding of the military lifestyle and experience working with military family members.
- Working knowledge of relevant military, state, federal, and local laws and resources.
- Highly developed written, oral, and presentation communication skills.
- Possession of, or familiarity with, the following certifications is not required but highly preferred:
- Emergency Clinical Obstetrics (ECO) or Advanced Life Support in Obstetrics (ALSO)
- Sugar, Temperature, Airway, Blood pressure, Lab work, Emotional support (STABLE)
- An active Security Clearance is preferred.
- Maintain a level of productivity comparable with that of other individuals performing similar services.
- Comply with the standards of The Joint Commission, applicable provisions of law, and the rules and regulations of any and all governmental authorities pertaining to licensure and regulation of health care personnel and medical treatment facilities, the regulations and standards of medical practice and the MTF medical staff bylaws.
- Adhere to and comply with the Department of the Navy, Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract.
- Participate in peer review and performance improvement activities.
- Practice aseptic techniques as necessary; comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection.
- Function with an awareness and application of safety procedures.
- Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.
- Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.
- Maintain an awareness of responsibility and accountability for own professional practice.
- Maintain DoD email account as directed by government supervisor.
- Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities.
- The HCW shall comply with the HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility. Providers shall obtain/maintain a National Provider Identifier (NPI) in accordance with DOD and MTF policy/instruction, as applicable.
- Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required.
- Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies.
- Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook.
- Be capable of performing a full range of medical services commensurate with the professional license held. Fluently and proficiently read, write, speak and understand the English language. The HCW must be able to provide contract company with proof of being able to fluently and proficiently read, write, speak and understand the English language if English is not the contractor’s first language.
