Case Manager - Behavioral Health Care Coordination (Greenbrook)

• →Guide patients through their treatment journey, providing a seamless and supportive experience from consultation to completion
• →Coordinate appointments, provider schedules, authorizations, specialty pharmacy activities, and treatment readiness requirements
• →Monitor patient progress, address barriers to care, and collaborate with cross-functional teams to keep treatment plans on track
• →Maintain accurate documentation, financial estimates, and patient records within CRM and EHR systems
• →Respond to patient inquiries with empathy, professionalism, and urgency while delivering exceptional service
• →Support high-quality patient outcomes while ensuring compliance with operational, coverage, and treatment requirements

• High School Diploma or equivalent required combined with 3-5 years of experience in clinical case management, patient care coordination, direct patient care, or a related healthcare setting
• Experience using EHR systems and Microsoft Office applications (Outlook, Word, and Excel)
• Strong communication, organizational, and patient service skills with the ability to manage multiple priorities

• Associate's or Bachelor's degree in a healthcare-related field, with experience in patient care coordination, case management, or healthcare operations
• Experience with CRM systems (Salesforce preferred) and knowledge of insurance benefits, prior authorizations, healthcare scheduling, specialty pharmacy, or Buy & Bill processes
• Experience in behavioral health, specialty healthcare, TMS, Spravato®, or other specialty treatment programs preferred

• Passionate about delivering exceptional patient care and customer service with empathy and professionalism
• Highly organized and detail-oriented, with the ability to manage multiple priorities in a fast-paced environment
• A proactive problem-solver who takes ownership and finds effective solutions
• Collaborative, adaptable, and quick to learn new systems, processes, and ways of working

Benefits Statement: Selected candidate will be eligible to participate in the Company’s employee benefit plans, which currently include: health insurance; dental insurance; vision insurance; optional supplemental medical insurance; health savings and flexible spending accounts; company-paid life insurance and additional voluntary life insurance; company-paid short term disability and long term disability insurance; participation in a 401(k) savings and investment plan with company contribution; and paid time off for non-exempt salaried employees is calculated as .625 per pay period worked for vacation time and .29166 per pay period for personal time. Full-time exempt employees will be eligible for Flex-PTO, which allows time off without accrual.
