~1 min read- →Accountable for overall treatment planning efforts, care conferences with families and providers, family and provider care updates, psychosocial needs and completion.
- →Coordinate and facilitate the development of a discharge plan of care for the patient population which includes accountability for the complex discharge process.
- →Ensure the experience of care for the patient and family is exceptional.
Requirements
~1 min read- Education: Master's in Social Work (MSW), Counseling, Psychology, or related human services field is required
- LCSW, LPC, or LMHC provisionally license is preferred
- Minimum of 2 years of clinical experience in a behavioral healthcare or acute psychiatric setting preferred.
- Previous experience in an acute in-patient behavioral health hospital a plus
- Experience developing and administering treatment plans
- Individual and group counseling techniques
- Exceptional communication skills
