Responsibilities

→Provide primary medical care services in home, facility, and telehealth settings
→Conduct physical exams, wellness visits, and manage chronic and acute conditions
→Order and interpret diagnostic tests and labs
→Prescribe and manage medications and individualized care plans
→Coordinate care with interdisciplinary teams and community partners
→Promote preventive care, patient education, and continuity of care
→Participate in after-hours or on-call rotation as needed
