• →Provide comprehensive primary care to patients across the lifespan
• →Conduct physicals, wellness visits, acute visits and follow-up care
• →Evaluate, diagnose and manage acute and chronic conditions
• →Provide preventive health counseling and patient education
• →Coordinate care with specialists and other healthcare team members
• →Utilize telemedicine when appropriate
• →Maintain accurate and timely electronic medical records

• →Current New York State Nurse Practitioner or Physician Assistant license
• →Graduate of an accredited NP or PA program
• →Family medicine or primary care experience preferred
• →Strong clinical, communication and interpersonal skills
• →Commitment to patient-centered, compassionate care
