Family Medicine or Med-Ped Physician

Burnt Hills Pediatrics and Internal Medicine, part of Community Care Physicians (CCP), is a well-established primary care practice providing compassionate, evidence-based care for patients of all ages. Our physicians are trained in Pediatrics or are dual board-certified in Internal Medicine and Pediatrics (Med-Peds), offering a unique ability to care for both children and adults within the same practice.

Position Summary

We are seeking a full-time Physician to join our collaborative primary care team. This role is ideal for a provider interested in caring for patients across the lifespan, with flexibility for those trained in Med-Peds, Internal Medicine, or Family Medicine.

• →Provide comprehensive primary care services to patients of all ages (pediatric through adult).
• →Diagnose and manage acute and chronic medical conditions.
• →Perform preventive care, wellness visits, and age-appropriate screenings.
• →Deliver continuity of care across life stages, supporting both individual and family health needs.
• →Collaborate with physicians, nurse practitioners, and physician assistants to ensure coordinated care.
• →Utilize electronic health records, telehealth, and patient portal tools to enhance access and efficiency.
• →Participate in quality improvement initiatives and practice development efforts.
• →Educate patients and families on health promotion, disease prevention, and treatment plans.
