Responsibilities

→Provide comprehensive pediatric primary care to patients from infancy through adolescence
→Perform well-child examinations, acute/sick visits, and follow-up care
→Assess, diagnose, and develop treatment plans in collaboration with supervising/collaborating physicians as appropriate
→Educate patients and families on preventive care, development, nutrition, and health maintenance
→Coordinate care with specialists and other members of the healthcare team
→Maintain accurate and timely electronic medical records

Community Care Physicians is a physician-led, independent multispecialty medical group serving the Capital Region. CCP provides the resources, technology, infrastructure, and support needed to allow providers to focus on what matters most—caring for their patients—while maintaining the collaborative feel of a community-based practice.

License and active registration as a physician assistant or registered nurse/nurse practitioner in New York State.
- Physician Assistant:
  - Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
  - Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
  - Current certification by National Commission on Certification of Physician Assistants.
  - Current CPR certification.
- Nurse Practitioner:
  - Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
  - Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
  - Successful completion of pharmacology coursework acceptable to NYSED.
  - Current CPR certification.
