Our Community Care Internal Medicine Niskayuna office is looking for a full-time an RN to join their team! This is a full-time position. The hours are: Monday-Friday, 8:00AM-4:30PM.

Responsibilities

→Document verbal conversations with patients when appropriate.
→Document information into patient record in an accurate, legible and timely manner.
→Assess vital signs and the reason for each patient visit.
→Triage patient phone calls: collect, analyze and create a well-documented plan of care for the patient based on their presenting symptoms and expected healthcare outcome.
→Support care management activities including coordination of patient needs, follow-up and connection to resources.
→Assist with transitions of care - coordinating post-discharge follow-up, medication reconciliation and communication with patients and care teams.
→Prepare patient for visit as appropriate, based on office/provider requirements.
