We are seeking a highly organized and detail-oriented Scheduler to coordinate patient visits for our Hospital at Home Program. This role manages patient referrals, schedules Nurses and Therapy staff, maintains accurate patient records, and ensures timely appointment coordination while serving as the main liaison between the hospital, patients, and clinicians. As the program grows, the role will also support broader home care and after-hours scheduling, requiring excellent organizational, communication, and multitasking skills in a fast-paced healthcare environment.
Responsibilities

- Match caregivers or nurses with clients based on compatibility, skills, and availability
Ensure timely and appropriate caregiver replacements when necessary
- Maintain positive and respectful relationships with clients and caregivers
Review nurse notes for quality assurance
- Coordinate with RN Care Manager regarding changing conditions and needs of clients
- Provide feedback to nursing staff on changes in condition or initial case briefings
Review client documentation and assist with data entry
Follow up with other vendors (such as MD orders, authorizations, etc.)
Create and maintain client, nurse, or caregiver schedules
- Adjust schedules as needed to accommodate client requests and unexpected events on Alora and ABS
- Monitor caregiver and nurse attendance and address scheduling conflicts promptly
Follow up with staff on clocking in/out inconsistencies
- Assist with system adoption by providing basic tech support user instruction
- Answer new patient intake calls using the HELP method and enter them into the ABS system
Answer incoming calls in a professional and courteous manner
- Identify the nature of each call and direct it to the appropriate department or individual
Provide helpful information and assistance to callers when necessary
Regularly update clients on caregivers/nurses about their schedules
Handle client inquiries and concerns with professionalism and empathy
Document scheduling changes and maintain accurate records
- Conduct regular follow-ups with clients, caregivers, or nurses to ensure satisfaction
- Identify areas for improvement and work with the management team to enhance service quality
- Ensure all scheduling and coordination activities adhere to relevant industry regulations and company policies
- Maintain client, nurse, or caregiver records, including contact information, preferences, and care plans Confidentiality
- Proven experience in scheduling, coordination, or a similar role, preferably in the healthcare or homecare sector
Strong organizational skills and attention to detail
Excellent communication and interpersonal skills
Compassion and empathy towards clients and caregivers
Proficiency in computer software and scheduling tools
Knowledge of healthcare regulations and compliance is a plus
