We are seeking a compassionate and driven Nurse Practitioner to join our growing Clover Home Care Team in Georgia.
You will provide care across the Savannah, Athens, Rome, Macon, Augusta, Thomasville, Chatham, Columbus, Brunswick, and Atlanta markets.
This is an opportunity to build meaningful relationships with patients while improving outcomes for medically complex and vulnerable senior populations.
Responsibilities
~1 min read- →Conduct in-home and telehealth visits for Medicare Advantage members.
- →Perform comprehensive assessments, medication reviews, and chronic disease management.
- →Develop and implement individualized care plans that address both clinical and social determinants of health.
- →Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.
- →Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic, member-centered care.
- →Leverage Clover Assistant and EMR technology to support proactive care management and close gaps in care.
- →Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations.
- →Educate members and caregivers on disease management, preventive care, and treatment plans.
Requirements
~1 min read- Active Nurse Practitioner (NP) license in Georgia or ability to obtain licensure.
- You have an active DEA registration.
- National board certification as a Nurse Practitioner.
- Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.
- Strong clinical assessment and chronic disease management skills.
- Passion for serving vulnerable senior populations.
- Home care, home health, palliative care, geriatrics, or population health experience preferred.
- Comfortable utilizing technology and telehealth platforms to enhance patient care.
- Excellent communication and collaboration skills.
- Experience in home-based care, house calls, home health, or population health management.
- Experience managing patients with complex and chronic medical conditions.
- Comfort with telehealth technology and hybrid care delivery models.
