JOB TITLE: Case Manager-Medication Assisted Treatment
SCEDHULE: Full Time, 6am-3pm
LOCATION: Rocky Point, NC Pender County
SUMMARY OF POSITION:
Supports Opioid Treatment Program in managing daily responsibilities of patients receiving medication assisted treatment. This position will be responsible for assisting in coordination to clinical care and medical team, maintaining ethical documentation of patient interaction, providing support to patients, reviewing treatment plans, and facilitating group therapy for patients receiving medication assisted treatment. Provides therapy if appropriately credentialed under necessary supervision. Early morning hours are required.
REQUIRED EDUCATIONAL/EXPERIENTAL QUALIFICATIONS:
- Meets requirements under 10A NCAC 27G.0104 as a Qualified Professional
- Registration with NCSAPPB as CDACI/CADC
- One year or more of experience working within medication assisted treatment programs preferred
POSITION HIGHLIGHTS:
- Maintains clients' records by reviewing case notes, logging events and progress.
- Monitors cases by verifying clients' attendance and requested urine drug screens; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Communicates clients' progress by participating in weekly interdisciplinary meetings; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
- Coordinates care to clinical and medical team by reviewing with patients, scheduling needed appointments, monitoring urine drug screen results, reducing barriers to participation through referral and linkage to community supports
- Maintains ethical and professional standards, acting in accordance with scope of practice as related to certifications and under direction of supervisor
- Acts under direction of clinical and administrative supervisor.
- In providing services, the employee may deal with a variety of collateral issues such as finances, medical, vocational, health, and behavior.
- Provide information to and confer with interdisciplinary treatment team
- Subject to inside environmental conditions and exposed to fumes, odors, dusts, mists, gasses or poor ventilation and subject to work in close quarters. Employees may be exposed to human body fluids and are covered by the OSHA rules and regulations on blood borne pathogens
