Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans
Administer medications, treatments, and skilled nursing procedures in a home health setting
Monitor and document patient progress, vital signs, and changes in condition
Coordinate care with physicians, therapists, and other healthcare team members
Educate patients and family members on diagnoses, medications, and home care techniques
Ensure compliance with all state, federal, and agency regulations and standards of care
Provide specialized nursing interventions as needed

Requirements:

Active Registered Nurse (RN) license in the state of Maryland
Previous home health, hospice, or community nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Reliable transportation and valid driver's license
CPR/BLS certification required
Compassionate, self-motivated, and able to work independently
