Key Responsibilities

As a ComForCare RN, you will lead high-quality, patient-centered care in the home setting by:

Comprehensive Nursing Assessments: Evaluate client physical, psychosocial, and environmental status to baseline needs.
Individualized Care Planning: Develop and update evidence-based care plans tailored to individual client goals and health outcomes.
Clinical Interventions & Oversight: Deliver targeted nursing interventions, administer medications as needed, and monitor client progress.
Care Coordination: Collaborate with family members, caregivers, physicians, and interdisciplinary healthcare teams to adjust care plans and initiate specialized referrals.
Documentation & Safety: Maintain accurate, timely clinical records adhering to safety, infection control, and HIPAA privacy guidelines.

Qualifications

Education: Graduate of an accredited School of Nursing.
Licensure: Active, unencumbered Registered Nurse (RN) license in Colorado..
Certifications: Current Basic Life Support (BLS) / CPR certification.
Requirements: Access to reliable transportation and valid driver's license. At least 1 year of home healthcare experience.
Attributes: Strong clinical judgment, excellent communication skills, and a passion for 1-on-1 personalized patient care. Oasis documentation
