Responsibilities

As one of Daymark’s Nurse Practitioners, you’ll work with our care team to implement our cancer care model with patients in their homes. Your primary goal is to provide great patient care within the home or virtually, whichever is needed. You'll also implement and help to continuously improve processes that will enable Daymark to be effective in Rhode Island and expand into new markets, build a highly engaged patient population, and contribute to a high-performing clinical team. You must be comfortable operating as a player/coach, rolling up your sleeves, and doing whatever is required to provide high-quality clinical care and to grow quickly. You’ll work closely with our clinical operations team, leveraging your expertise to establish our evidence-based oncology care and navigation model to address clinical (symptom management, in-home urgent care, palliative care, mental and behavioral health care) and non-clinical (social and related needs).   

  • Provide clinical care: You’ll provide direct patient care both in home and via virtual platforms. You’ll conduct assessments, place orders, prescribe medications, document appropriately, and assign tasks to enable our broader care team to deliver great patient care.     
  • Enable Daymark’s ability to triage for high acuity: You’ll help us provide navigation and direct clinical care for our highest acuity patients while also supporting our broader patient population. You’ll collaborate closely with oncology practices in our market through daily standups with their own clinical leadership teams.  
  • Real-world application of a new care model: You’ll collaborate with Daymark’s clinical operations team to refine and implement our care model. You’ll also help us deliver the right care at the right time to patients with a cancer diagnosis. 
  • Expertise: You are an active NP or PA and have been practicing for at least 4 years, delivering direct clinical care to patients. Oncology, home care, palliative care or primary care experience required. Oncology Nursing Society certification, experience with value-based care and/or in home care are each a plus.  
  • Tech savvy: You are comfortable caring for patients with cancer both in-person and virtually over video and web-based call software.
  • Analytical Skills: You can roll up your sleeves and contribute to clinical problem solving.  
  • Biased to action: You know how to identify and prioritize your initiatives and ensure that urgent and important tasks always get done. 
  • Comfort with ambiguity: You have experience being a part of building or launching new programs and are comfortable with changing processes to improve patient care. 
  • Player/Coach: You can lead your team and can care for patients and switch contexts with ease, as all will be required in a day’s work. 
  • Able to work in-person. You'll deliver in-home care to patients in Rhode Island and potentially Southeastern Massachusetts.
  • Availability: You can work with our team and patients full-time, four days per week. 

The expected compensation for this role is $140,000 to $170,000 annually, depending on experience. 

Our office is based in Providence, RI and you will deliver in-home care to patients in RI and potentially Southeast MA. Candidates must have a valid driver's license and a reliable personal vehicle.
