• →Coordinate patient-centered care activities using Ennoble Care’s electronic medical record (EMR) system
• →Support Ennoble Care providers and clinical teams through accurate documentation, follow-up tracking, and care coordination
• →Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
• →Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
• →Assist with care transitions, referrals, and coordination with external partners, as needed
• →Collaborate closely with fellow Care Coordinators and internal care team members within an office-based environment
• →Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision-making and efficient clinical operations
• →Contribute positively to team collaboration, workflow efficiency, and day-to-day office operations

• Legally authorized to work in the United States of America
• Active, unrestricted clinical certification or licensure, such as:
• Patient Care Technician (PCT)
• Certified Nursing Assistant (CNA)
• Medical Assistant (MA) / Certified Medical Assistant (CMA)
• Licensed Practical Nurse (LPN)
• Registered Nurse (RN)
• Or comparable credential
• Minimum 1 year of experience in a clinical and administrative or care coordination role within a healthcare setting
• Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems
• Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
• Well-developed organizational, time-management, and attention-to-detail skills, paired with an optimistic approach and initiative-driven mindset
• Strong working knowledge of healthcare terminology and care coordination principles
• Proficiency with Microsoft 365, including Outlook, Word, and Excel
• Ability to work full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST, in an office-based setting in the Voorhees, New Jersey area
• No weekends
• No on-call
• Daytime hours
• Seven (7) company-paid holidays annually
• Ability to pass a background check and drug screen
