As a Vice President of Clinical Operations with Evergreen Nephrology, you are responsible for providing senior leadership for enterprise clinical programs with direct accountability for clinical quality performance, HEDIS/ Stars execution, and value-based care outcomes in clinical operations. This role oversees clinical programs across the continuum, including post-acute care optimization, compassionate care and palliative pathways, and CKD navigation.
The VP is responsible for translating clinical priorities into scalable operating models supported by data, technology, and clear workflows. This role focuses on improving outcomes, meeting our quality metric performance goals, reducing total medical expenditure, and ensuring patients receive the right care at the right time in the right setting.
Responsibilities
~1 min read- →CKD Navigation, PAC, ECCP, and future clinical program performance
- →Ensure clinical programs align with VBC contract requirements
- →Clinical program stabilization to drive performance and scale
- →Serve as senior escalation point for program risks, performance gaps, and complex operational challenges
- →Partner with our Central Quality leadership team to translate enterprise quality strategy into program-level execution, workflow design and front-line accountability
- →Drive data integrity and reporting alignment across EG tech platforms and reporting systems
- →Advance tech enabled, omni channel care delivery to improve pt engagement, documentation accuracy and measurable outcomes
- →Own program level KPI performance with rapid cycle improvement methods to close performance gaps
- →Build and develop high performing clinical leadership teams with clear role clarity, performance expectations and accountability structures
- →Other duties consistent with this role, as assigned.
Requirements
~1 min read- Experience leading clinical operations in Value-Based Care
- A clinical degree with experience in direct patient care and clinical operations - backgrounds may include nursing, NP/PA, pharmacy, social work, or medicine
- Experience and demonstrated ability to partner effectively with physician leaders and build strong professional relationships
- Strong working knowledge of HEDIS and Stars performance mechanics
- Direct experience with care management platform implementation and optimization
- Ability to build oversight structures across centralized and embedded teams
- Proven ability to manage and meet outcomes in a fast-paced environment
- Track record of building disciplined operating systems and clear accountability
- Experience leading through influence in matrixed, growth-oriented environments
- Intermediate skills with MS Office Suite of products including Outlook and Teams
- Able to work effectively in a primarily remote environment:
- Home internet must support a minimum download speed of 25 Mbps and upload speed of 10 Mbps. Cable, Fiber, or DSL connections hardwired to the internet device are recommended
- Evergreen will provide remote employees with telephony applications and equipment to meet the business requirements for their role
- Employees must work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Requirements

What We Offer
~1 min readThe pay range for this role is $200,000 to $220,000 annually. Exact pay is determined based on experience, education, demand for the role, and other role-specific factors.
This role is also eligible for an annual bonus.
