Job Title: Speech-Language Pathologist (SLP) – Home Health

Reports To: Director of Nursing



Job Summary:

The Speech-Language Pathologist (SLP) provides skilled speech and language therapy services to adult and geriatric patients in their homes. The SLP evaluates, diagnoses, and treats speech, language, cognitive-communication, voice, and swallowing disorders to promote optimal recovery, independence, and quality of life.



Key Responsibilities:

  • Perform initial and ongoing speech therapy evaluations in the patient's home.
  • Develop and implement individualized care plans based on patient assessment and physician’s orders.
  • Provide therapeutic techniques for the rehabilitation of patients with speech, language, voice, cognitive, and/or swallowing disorders.
  • Instruct patients, families, and caregivers in therapy techniques and strategies.
  • Collaborate with interdisciplinary team members (nurses, occupational therapists, physical therapists, etc.).
  • Maintain accurate and timely documentation in accordance with state and federal regulations, payer requirements, and company policies.
  • Educate patients and families about goals and progress.
  • Reassess and adjust care plans as necessary.
  • Adhere to infection control and safety policies during home visits.

Qualifications:

  • Current, active license as a Speech-Language Pathologist in the state of Florida .
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Minimum of 1 year experience in a clinical or home health setting (preferred).
  • CPR certification (required)..

  • Strong communication and interpersonal skills.
  • Ability to work independently in a home health setting.
  • Organizational and time management skills.
  • Cultural sensitivity and the ability to work with diverse patient populations.
