Join our team and make a positive impact on the lives of others! We are looking for a Home Care Supervisor, RN for our Colorado team! 

• →Home care case management 
• →Care coordination experience 

Qualifications 

• →Valid and active Registered Nurse (RN) license in the State of Colorado.  

• →Ability to articulate cases through effective case summaries. 

• →Ability and flexibility to travel (usually by car) within assigned geographic area; valid driver’s license and car insurance required. 

• →Proficient in common software and web-based applications (e.g. Case Management Systems, MS Office) with the ability to teach others. 

• →A passion for providing a high level of customer service, including the ability to show empathy, active listening, and patience.
