You’ll be the first face families see as they begin their hospice journey. From heartfelt conversations to creating personalized care plans, you’ll lead the way in making sure every patient and family feels supported, informed, and cared for.

Responsibilities

Compassion Meets Action: Perform comprehensive assessments, guide care plans, and work directly with physicians to ensure each patient’s needs are met.
Documentation Extraordinaire: Keep your notes sharp—EMR documentation within 24 hours!
On the Move: Travel to patient homes (yes, this includes navigating stairs and weather surprises) to deliver exceptional care. Must be willing to travel to Orange County and Inland Empire. 
Family Whisperer: Educate, comfort, and empower families during their hospice journey.
Teamwork Makes the Dream Work: Collaborate with our interdisciplinary team to achieve positive outcomes.

Graduate of an accredited nursing program.
RN License (active and in good standing).
1+ years of nursing experience (hospice experience required).
A valid driver’s license, insured vehicle, and a sense of adventure for the road ahead.
Bilingual (Spanish/English)? Amazing!
