Registered Nurse - RN Case Manager - Hospice

We are currently seeking compassionate, skilled Registered Nurse Case Managers to join our team. This is a combined job posting for two distinct opportunities within our organization: Hospice RN Case Manager and Home Health RN Case Manager. The Hospice Registered Nurse Case Manager participates as a member of the Interdisciplinary Team (IDT) to provide holistic care to patients and families. This role is responsible for comprehensive assessments, care coordination, and ongoing evaluation of patient and family needs. The RN ensures implementation and ongoing updates of the interdisciplinary plan of care to support comfort, dignity, and quality of life at end of life.

• →Provide case management for assigned hospice patients
• →Conduct comprehensive assessments and ongoing visits
• →Develop, implement, and update individualized Plans of Care with the IDT
• →Manage pain and symptom control in collaboration with the physician
• →Educate patients and families on medications, symptom management, and end-of-life care
• →Coordinate care with physicians, interdisciplinary team members, and caregivers
• →Provide skilled nursing interventions including wound care, IV therapy, and symptom management
• →Participate in IDT meetings, on-call rotation, and quality improvement initiatives

• Graduate of an accredited nursing program
• Minimum of 2 years nursing experience (home health or hospice experience, acute care, or geriatrics preferred;
• Active Virginia RN license (or compact license)
• Current BLS/CPR certification or obtained within 30 days of hire
• Strong clinical assessment, communication, and decision-making skills
• Comfort with EMR systems and mobile documentation tools
• Valid driver’s license, reliable transportation, and proof of insurance
