Weekend RN - Hospice

• →Provide weekend on-call coverage and respond promptly to patient and caregiver phone calls, medical or emotional crises, and patient deaths
• →Conduct comprehensive nursing assessments and complete admissions as needed
• →Make home or facility visits to address urgent patient care needs and provide direct care when appropriate
• →Communicate effectively with attending physicians, GSH staff, and other agencies to coordinate optimal patient care and resources
• →Educate and support patients and families regarding hospice philosophy, disease progression, symptom management, stages of dying, and the plan of care
• →Collaborate with the Interdisciplinary Team (IDT) by reporting changes in patient status and communicating important patient and caregiver concerns
• →Assist RN Case Managers with scheduled after-hours visits and patient care needs to ensure continuity of care
• →Complete timely and accurate documentation of all patient interactions, visits, assessments, and care provided in the electronic medical record
• →Provide a thorough report to the Interdisciplinary Team following each shift
• →Maintain clinical competencies and participate in required staff meetings, in-services, and continuing education

• Clinical Expertise – Graduate of an accredited nursing program with current Virginia RN licensure
• Experience – Minimum 2 years in nursing, including hospice, home health, med/surg, acute care, or geriatric nursing
• Critical Thinking & Decision-Making – Strong judgment, problem-solving, and ability to handle complex patient situations
• Communication Skills – Excellent verbal, written, and interpersonal skills; able to work collaboratively with the IDT
• Technology Skills – Experience with EMR documentation and general computer applications
• Compassion & Professionalism – Provides patient-centered care with empathy, discretion, and integrity
