Maintain the department’s master patient list and/or tracking systems, including the timely addition of newly identified patients to support appropriate clinical follow-up, care coordination, and continuity of care. Ensure patients requiring additional review or higher levels of clinical management are appropriately identified and reflected on applicable tracking lists.
Serve as a primary point of contact for missing clinical documentation, including outreach to external healthcare facilities and other providers to obtain necessary reports, test results, and medical records when information is not readily available within the organization’s electronic health record.
Conduct patient outreach regarding outstanding tests, appointments, referrals, or other required follow-up, and assist with coordinating scheduling to support completion of identified care needs.
Coordinate outreach for required patient consent forms and other documentation, ensuring completed forms are appropriately documented and reflected in applicable departmental tracking systems.
Maintain accurate and up-to-date patient records, tracking tools, and departmental documentation to support effective care coordination, clinical workflows, and continuity of care.
Maintain and update required care plans, patient-specific documentation, and other departmental records within the electronic health record, ensuring corresponding information is accurately reflected in applicable tracking systems.
Conduct outreach to patients who have transferred care to another provider or healthcare organization and update departmental and electronic health records accordingly.
Establish and maintain appropriate episodes of care, encounters, or other required electronic health record documentation for patients transferring into the department. Enter, reconcile, and organize relevant clinical information received as part of transferred medical records, including diagnostic reports, laboratory results, imaging, and other pertinent documentation.
Assist with identifying and tracking patients who may require additional clinical review, follow-up, or care coordination and ensure appropriate information is available to the clinical team for management.
Perform Medical Assistant duties as needed to support clinical operations, including rooming patients, obtaining and documenting vital signs and patient information, preparing patients for visits, and assisting clinical staff with patient flow and other duties within the scope of the Medical Assistant role.
Monitor departmental inventory of patient education materials, informational packets, forms, and other supplies, and coordinate ordering and replenishment as needed.
Communicate and collaborate with clinical staff, administrative staff, external healthcare organizations, and other departments to facilitate the timely exchange of information and support coordinated patient care.
Support departmental quality, documentation, and workflow initiatives by maintaining accurate records, identifying outstanding items, and assisting with follow-up as appropriate.
Maintain patient confidentiality and handle protected health information in accordance with organizational policies, procedures, and applicable privacy requirements.
Perform additional duties and responsibilities as assigned by department management in support of patient care, departmental operations, and organizational needs.
