About the Role:
ComForCare Home Care - Clarksville is looking for a compassionate and organized Care Coordinator to join our growing team in Hendersonville,  TN. In this role, you'll be the vital link between clients, caregivers, and families — ensuring every individual receives the highest quality of in-home care. If you're passionate about making a difference in people's lives, we want to hear from you!

Must be willing to work part-time care coordinator 50% in the office and as a caregiver 50% in the field with our Clients.

Responsibilities:
  • Coordinate and schedule caregivers to match client needs, preferences, and care plans
  • Serve as the primary point of contact for clients, families, and caregivers regarding care services
  • Conduct client assessments and develop individualized care plans in collaboration with the care team
  • Monitor caregiver performance and ensure compliance with home care standards
  • Manage caregiver scheduling, shift changes, and on-call coverage to maintain continuity of care
  • Maintain accurate client and caregiver records in compliance with regulatory requirements
  • Assist with caregiver onboarding, training, and ongoing support
  • Respond promptly to client concerns and resolve service issues with professionalism and empathy
Requirements:
  • Prior experience in home care, healthcare coordination, or a related field preferred
  • Strong organizational and multitasking skills in a fast-paced environment
  • Excellent communication and interpersonal skills with clients, families, and staff
  • Proficiency with scheduling software and basic computer applications
  • Ability to problem-solve quickly and remain calm under pressure
  • Compassionate, patient-centered mindset with a commitment to quality care
  • High school diploma or equivalent required; associate's or bachelor's degree in a health-related field a plus
