Position Summary: The Priority Care Coordinator (PCC) facilitates the coordination of high-quality, patient-centered care for a select group of patients under the guidance and supervision of licensed clinical staff. The PCC works closely with Nurse Case Managers and other interdisciplinary team members to identify, assess, and address patient needs, ensuring continuity of care and positive health outcomes.


Essential Position Functions/Responsibilities:
  • Conducts daily outreach to patients, physicians, and family members for health screening, assessment, intervention, and follow-up as directed by the Priority Care Nurse Case Manager (PCCM).
  • Identifies potential transition-of-care needs and high-risk patient situations in collaboration with the PCCM.
  • Conducts initial telephonic assessments for members referred to the PCC program and documents findings in the case management module.
  • Participates in patient rounds, case reviews, and meetings with medical leadership as needed.
  • Collaborates with PCCM to prioritize and plan interventions for high-risk patients with conditions such as congestive heart failure, diabetes, cancer, asthma, COPD, and other complex diagnoses.
  • Interfaces with community agencies, home care providers, and DME vendors to facilitate patient care.
  • Acts as a liaison to Provider Relations and participates in provider network presentations as requested.
  • Participates in external health and wellness programs coordinated by the organization’s marketing or clinical leadership.
  • Provides support for onboarding and training new staff on program processes as assigned.
  • Uses independent judgment to manage high-volume workloads, prioritize tasks, and meet deadlines while maintaining quality and patient-centered care.
  • Participates in special projects or initiatives as directed by management.

Qualification Requirements:
Skills, Knowledge, Abilities
  • Excellent verbal and written communication skills.
  • Strong organizational, problem-solving, and time management abilities.
  • Ability to manage multiple tasks and projects simultaneously.
  • Team-oriented with a sense of urgency and accountability.
  • Proficient in Microsoft Office applications (especially Excel) and other computer systems.
  • Knowledge of case management processes, managed care operations, and patient-centered care coordination.

Training/Education:
  • Associate’s Degree required; Bachelor’s Degree preferred.
  • Clinical or medical experience required.
  • Experience with patient and family education, case documentation, and healthcare coordination.
  • Knowledge of Priority Care coordination and quality management concepts preferred.
Experience:
  • Prior experience in a clinical or medical setting, ideally within a managed care or case management environment.
  • Familiarity with patient assessment, care planning, and care transition processes.
  • Experience collaborating with interdisciplinary teams and external health care partners.

Base Compensation; $45,000 - $55,000 (21.63 - $26.44 per hour)
