This fully remote role supports patients navigating complex kidney health needs across the continuum of care. As a Care Navigator, you will manage a diverse caseload, coordinate services, and help patients understand their treatment and care options. You will combine clinical expertise with patient advocacy to promote high-quality, cost-effective outcomes. The role involves collaborating closely with physicians, nurses, families, and other healthcare professionals to address barriers and achieve individualized goals. You will also support medication management, health assessments, care planning, and ongoing follow-up. Candidates must be able to work virtually across Pacific and Mountain time zones, with no travel required.
- Manage a caseload of members with varying levels of clinical complexity, conducting appropriate follow-up according to established policies and care cadences.
- Conduct inbound and outbound calls to engage members, provide education, and deliver a high-quality care navigation experience.
- Educate patients about kidney health, related conditions, dialysis options, transplant pathways, and strategies for optimizing renal replacement therapy.
- Complete health and social determinants of health screenings, medication reconciliation, care plans, and documentation of identified needs, barriers, interventions, and goals.
- Advocate for patients by identifying and resolving concerns, connecting them with appropriate community resources, and supporting self-management.
- Coordinate care as part of an interdisciplinary team, serving as a liaison between patients, families, physicians, and ancillary healthcare providers.
- Communicate identified patient needs to providers and support patients and families in taking an active role in their care.
- Maintain accurate documentation in care management systems while complying with HIPAA, regulatory requirements, operational procedures, and security policies.
- Track individual performance metrics and key performance indicators while effectively navigating tools such as Excel, OneNote, Outlook, and Word.
- Provide support across different time zones, including after-hours coverage when required by business needs.
Requirements

- Bachelor’s degree required; candidates with 6+ years of RN experience, including at least 3 years in case management, may be considered in lieu of the degree.
- At least 3 years of case management experience preferred.
- Certified Case Manager (CCM) certification preferred.
- Experience working in a dialysis or transplant center is preferred.
- Familiarity with Medicare and Medicaid is preferred.
- Strong patient advocacy skills and the ability to foster collaboration, positivity, and motivation.
- Excellent verbal and written communication skills, with the ability to engage effectively with patients, families, providers, and interdisciplinary teams.
- Strong strategic thinking, planning, organization, and problem-solving abilities.
- Ability to adapt and remain effective in a challenging environment with changing priorities.
- Comfortable working with technology and multiple digital applications in a remote setting.
