About the opportunity:
The Northern Outreach team provides case management to people who are experiencing homelessness or are at risk of homelessness, with multiple and complex needs. This may include rough sleepers, couples, families and people aged 50 and above. The purpose of this program is to get people housed, keep people housed and work towards preventing further episodes of homelessness. The geographical area covered by the Northern Outreach Team is the City of Yarra, the City of Merri-Bek & the City of Darebin.
The service has multiple funding sources including Transition Support and Home and Community Care for Young People Program (HACC PYP).
The role of a Northern Outreach Case Manager is to increase the quality of life and social inclusion of clients. Service is delivered via an assertive outreach model and achieved by building a purposeful, therapeutic relationship and providing support in a practical and comprehensive way, including a recovery-based case plan.
Maximum-term, full-time (76 hours per fortnight) based at Collingwood, until 20 April 2027.

Working with clients in their homes, on the street, in rooming houses or other locations in the community to offer assistance and to make appropriate referrals to external agencies. Working with clients in regard to basic needs such as emergency accommodation, housing, income entitlements, employment, food, clothing, home support services and medical care. Working within a collaborative, strengths-based model with a strong client-focused perspective in the development of case plans. These are aimed at assisting the client to formulate their own journey of recovery.Accurately maintaining client files and records in accordance with organisational procedures on a computer case management software system. Identifying service gaps and fostering collaboration with other agencies.
A relevant tertiary education or demonstrated experience working in social services or outreach (mandatory).A valid Victorian driver’s licence (mandatory).Experience working with clients with complex needs and social policy/client support frameworks (highly desired).Demonstrated ability to build positive relationships and communicate with people of diverse backgrounds and abilities.Demonstrated ability to work collaboratively with others towards effective client solutions.Good interpersonal and communication skills, both verbal and written.Well-developed organisational, task prioritisation and time management skills.Demonstrated commitment to the values of Launch Housing and our mission to end homelessness
