• →Provide coaching and education using motivational interviewing, self-management education and relapse prevention strategies to support tobacco cessation

• →Utilize evidence-based techniques while providing one-on-one counseling to create an individualized plan of treatment
• →Assess and evaluate patients and develop plan of care integrating various nicotine replacement therapy
• →Provides assistance in developing effective coping strategies to keep clients tobacco-free
• →Attend and present at community education events as needed
• →Participate in continuous training and education to maintain quality standards

• Must have current Tobacco Treatment Specialist certification or be able to obtain certification after hire by attending tobacco treatment training
• Must possess a high level of interpersonal skills
• Minimum of Associates Degree, Bachelor's Degree preferred
• Familiarity with Microsoft Office Suite
• At least one year's experience in counseling, health education, or patient care preferred
• Must be nicotine free
• Excellent written and oral communication skills
• Must be an adaptable team player

• LMH is accredited by DNV and TJC, and as such, may require specific annual education related to specialty certifications and standards.
