Overview:
The Targeted Case Management Specialist plays a key role in connecting clients to essential medical and support services, focusing on individuals who speak English as a second language. Case management, as defined by the Ryan White Program, involves a range of client-centered services that link clients to healthcare, psychosocial support, and other necessary services, provided by both medically credentialed and other trained professionals. The Targeted Case Management Specialist will assess each client’s unique needs, develop personalized care plans, and ensure access to both formal and informal support systems. This role includes advocating for clients, promoting health literacy, and working closely with multidisciplinary teams to facilitate access to treatments, medications, and insurance. By building strong connections with clients, community partners, and support networks, the Targeted Case Management Specialist ensures clients receive the necessary resources and care, with a particular emphasis on addressing language and cultural needs.
Key Responsibilities
- Assess and identify the individual needs of clients, particularly those who speak English as a second language, providing tailored interventions and fostering patient-centered care.
- Advocate, support, and educate clients, with a focus on wellness and informed health decision-making, ensuring culturally sensitive approaches for clients who may face language barriers.
- Develop, implement, and manage an inclusive, individualized care plan that considers language needs, ensuring access to both formal and informal supports. Make necessary referrals to community resources.
- Promote health literacy by assisting clients in making informed decisions about their health, addressing any language or cultural barriers that may arise.
- Strengthen professional practice through continuous learning of best practices and skill development, particularly related to working with diverse, multilingual populations.
- Ensure clients receive certification/recertification for the Kentucky Ryan White Program and facilitate access to medication, treatment adherence counseling, and medical insurance, with special attention to language support needs.
QUALIFICATIONS
- Must have a Bachelor’s of Social Work or a B.S. in a related field.
- Bi-lingual is preferred
- Basic knowledge of case management principles.
- Experience working in a nonprofit organization or a strong desire to work in service driven environment.

- Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
- Must be able to pass a drug screen and background test.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work vary, but within Indiana are Monday, Wednesday, Friday 8-430, Tuesday 7-7, Friday 8-1230. Occasional evening and weekend work may be required as job duties demand.
This position is considered a hybrid position when duties are performed within the scope of this job description. If performing duties outside of the essential duties of this job description, the employee may request an alternative accommodation from their director to best meet the needs of the employee for the terms of the irregular duties. (Example: Employee is participating in a virtual conference and is requesting to WFH to attend and accommodate employee needs during the time of the conference).
