Transition Facilitator/Blended Case Manager

We are seeking a Blended Case Manager- Transition Team to join our team in our Cambria County, PA location.

Earn $19.33/hour! 

Under the direction of the Blended Case Management Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan. As a member of the BCM team this position will serve as primary contact for no more than 30 individuals. Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients’ needs along with supportive counseling, and will include on-call responsibilities.

• Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
• Registered nurse; or
• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years’ experience in direct contact with mental health consumers
